Pre-Registration with the practice
It is important to register with a Practice as soon as you move into the area - please don't wait until you need medical attention!
In order to register with the practice, check the map to ensure you are resident within our boundaries. There are some exceptions ie patients living at Chilton Hall and Cedars Park need to register with Stowhealth.
It would be helpful to us if before registering with the Practice, you could complete the New Patient Health Questionniare below.
The Receptionist will then ask you to complete an additional GMS1 (purple form) for each member of the family - this is only available in the surgery not online. If you have details of your previous GP, previous address and NHS number please bring them with you as this will help the registration process.

The Practice is open from 8am to 6.30pm on weekdays

Enhanced Summary Care Record
You can choose to have additional information included on your Summary Care Record which can enhance the care you receive. This includes;
- Your illnesses and health problems
- Operations and vaccinations you have had in the past
- How you would like to be treated, such as where you would prefer to receive your care
- What support you might need
- Who should be contacted for more information about you
